An interdisciplinary team of healthcare professionals work cooperatively to create individualized plans for each patient. Collaborative care is beneficial for patients with various needs such as physical, dietary, occupational and speech. With a disciplinary team led by a physician, patients are granted different perspectives, personalized care, treatment efficiency and better communication.
Collaboration among healthcare professionals makes for more efficient rehabilitation, improved care and outcomes, fewer errors, reduced length of stay and a clear end goal. The opportunity to share perspectives is essential in high-quality care.
Family members are encouraged to be involved in the care plan and progress of their loved one. Our team keeps family members involved in decisions as appropriate. In addition, they provide valuable information about the patient’s medical history, preferences and support system.
We take a collaborative approach for each patient in our care at Cascade to ensure improved quality of life and treatment efficiency. Once an individual is admitted to our facility, a physician is assigned. The physician writes orders for all necessary medications and works closely with a team of healthcare professionals in developing individualized care plans. Our top priority is care catered to your needs. This includes rehabilitation programs, special dietary needs and clinical treatments.
